When complete please mail or email your information back to our office.  Your trainer will then be able to complete your personal training program to best meet your health and activity goals.

Individual and Small Group Training is scheduled by appointment only.  Packages must be purchased in full at time of initial scheduling with a minimum of one half package sessions scheduled at that time.  Cancellations must be made 48 hours prior to scheduled session time.
